Demelza Community

The service complements the care offered at Demelza Kent and Demelza South East London, with the Community Palliative Care Team going directly into the family home to provide care, respite, an on-call service and crisis intervention.

Demelza Community provides care and support based on the individual needs of the family. This is achieved through an holistic assessment and development of a care plan in collaboration with the family and other healthcare professionals. The care plan will reflect the changing needs of the child/young person and will embrace the needs of the family as a whole.

Care is provided using a variety of skills and services and is co-ordinated by a named nurse within the Demelza Community Team. The team works in partnership with families to help support and maintain family life

When a child reaches the end of their life the Demelza Community Team can offer 24 hour end of life care at home followed by bereavement support to members of the family, as required.

Services provided include:
  • Support following diagnosis
  • Home visits, appropriate to the child/young person and the family needs
  • Nursing care, practical and emotional support in the home
  • The provision of information about treatment, medication and medical condition
  • Crisis intervention
  • Assessment for the need for specialised equipment and onward referral, where appropriate
  • Co-ordination of services provided in the home
  • Teaching, supervision and support for parents/carers wishing to administer the child/young person's care and treatment
  • Respite care in the home for child/young person with on-going healthcare needs
  • On call service from 8am until 10pm (07714 282232)
  • 24 hour support for child/young person requiring care in the terminal stages of their illness in conjunction with other healthcare professionals (07714 282232)
  • Liaison with professionals involved in caring for the family
  • Provision of information, advice, and support for children/young people, their families, carers, health professionals and other agencies
  • Supporting and advocating for families to ensure that their views are heard.

Demelza Commissioned Services

Demelza runs a commissioned service (funded by Health, Social Services or Education) providing packages of care for children/young people with complex healthcare needs. The package will support the child/young person's parents or carers to manage their care at home or in other settings such as school.

Packages of care are agreed after a comprehensive multi-disciplinary assessment as described in the Department of Health document 'National Framework for Children and Young People's Continuing Care' March 2010.
