The Family Support Team provides a wide range of services, such as:
• Timely referral to Talking & Creative Therapies and pre / bereavement support.
• Timely assessment of family's needs to identify appropriate services and resources.
• Advice and practical assistance with applications for equipment, short breaks, i.e. 'Dreams come true' events and benefits.
• Advocacy on behalf of young person or family, to assist with access to services or resources.
• Transition to support families and young people aged 16+ entering adult Health & Social Care services.
• Pre-Bereavement, Bereavement, One-to-One & Group Work with children and their families / significant others.
• Networking, communication and direct work with other Health & Social Care professionals, to facilitate access to identified services based on an assessment of needs.
• Participating in internal and external multi-disciplinary reviews, planning meetings and case conferences relevant to the child or young person.
• Adhering to Safeguarding procedures for Children / Young People and Adults at Risk.
Demelza Hospice Care for Children is an organizational member of the British Association for Counselling & Psychotherapy (BACP) and adhere to the Ethical Framework for all counseling skills based activities. All work undertaken is located with relevant clinical and organizational policy frameworks to ensure safeguarding. |
