| | | Short BreaksYoung People Palliative Care End Of Life Care | | Families may wish to let us care for their child while they stay at home or go away to 'do their own thing', or simply recharge their batteries. At other times, they may wish to come and stay with us as a family, using our comfortable family rooms and open plan living areas. Each stay is booked on an individual basis, so families can chop and change as suits their particular family circumstances at the time. If they choose to stay with us we are happy to do as much of the child's care as they want and that may change at each visit, too. To ensure we offer a consistent high standard of care to each individual child or young person, we have devised a care plan. Children and young people will be allocated a number of nights per year (in our hospice buildings), or care hours in the community, but will also be able to access our services as an emergency or to use our facilities on a day care basis. We request on the child's first residential short break that a parent or carer stays, so that we can be effectively introduced to the child or young person and learn about their individual needs. Demelza can offer specialist short breaks for children or young people who it is difficult to place elsewhere due to their frailty, vulnerability and/or complexity of medical and care needs. Demelza can also offer a 'stepped discharge' from a hospital setting where a child may have complex needs such as requiring ventilation. In the hospice the family can become confident and competent in providing the care their child will need when they go home. |
