Medical, clinical, social, educational and spiritual information will be gathered from all professionals involved, to assist us to best meet the needs of the child or young person.

It may not be possible to accept all children or young people referred to our service, but this will be discussed with those with parental responsibility and signposting to other services will be offered.

Each child or young person referred to our service will be annually reviewed to ensure that they still meet the criteria. Sometimes a child or young person's condition may improve and we will liaise with the family towards a discharge from our service. We will also work with any young person and their family who is likely to continue to need hospice or other services into adulthood.

Many children or young people referred to our service have complex medical and other needs, therefore there is no definitive list of conditions or illnesses which may or may not meet the criteria.
